Purchase order must contain the following information:
1. Shipping address
2. Billing address
3. Contact name
4. Contact phone number
5. Item name or number
6. Quantity of toys

If your order is to be shipped to NJ and tax exemption applies to your organization, please attach tax exempt certification to the purchase order.
